Dear Parents/Guardians:
I hope everyone’s summer is off to a great start! The Health Office is contacting parents and guardians of high school students, to remind them of NYS health requirements needed for the 23/24 school year.
Students entering grades 9 and 10 have no requirements, unless your student plays a sport, then an updated physical examination is required to play.
Students entering grade 11 are required to have a current Physical Examination on file. The physical examination form must be dated no later than September 1, 2022 and on the NYS HEALTH EXAMINATION FORM dated 5/2023 or an electronic equivalent.
Students entering grade 12 are required to have the 2nd Meningococcal vaccine in order to attend school. If you are unsure of your students’ immunization status, please call your doctor’s office and schedule an appointment if needed. if NYS required immunizations are not completed by Monday, September 25, 2023, a letter of exclusion maybe issued and your student can not return to school until proof of immunizations are received.

We also would like to take this opportunity to remind you that all medications require a doctor’s order on file in the health office. This includes over the counter medications and rescue medications, such as epi pen, inhaler, and glucagon. It is highly encouraged that all high school students that require rescue medications, obtain self-carry orders from their physician, if they approve, so they can safely participate in all activities including sports and after school clubs.
