Dear Parents and/or Guardians:
Thank you for a wonderful school year! I am contacting parents and guardians of DHS students, to remind them of NYS health requirements needed for the 24/25 school year.
We will be offering Fall ‘24 Sports Physicals over the summer on Monday, August 26th at 8am at DMS/ DHS Health Office. All athletes need a current physical in order to participate in sports. Physicals are valid for one year, till end of month. If you would like your student to have a physical on August 26th, complete and sign the included Permission form, and bring on the day of physicals. Please email me, to let me know your student will be coming. Physicals will be held 2 other times during the 24/25 school year, November and March.

Physical examinations must be completed on a REQUIRED NYS SCHOOL HEALTH EXAMINATION FORM || 5/2023 form or an electronic equivalent. If not, you will be asked to have the physician complete the correct form.
If you are unsure of you student’s immunization status, please contact your physician.
We also would like to remind you that all medications, including rescue and over the counter medications, require a doctor’s order allowing the medication to be carried by students and/or administered by the school nurse. There are no exceptions.
We suggest you make copies of the completed forms for your own records before sending them to the school health office. Forms may also be faxed to the number below or emailed to me. If you email, it may take 2 to 3 days to reply.
